Death Benefits
The following is a list of possible death benefits
1 Social Security Benefits
The Geiple Funeral Home, Inc. will promptly notify the Social Security Administration of the beneficiary's death through SSA-721. "Statement of Death by Funeral Director". The Local Social Security Office will be in contact with eligible survivors either by phone or mail. |  |

Possible Social Security Benefits:
A. One-Time Lump Sum Death Benefit
The Social Security Administration provides a "One-time lump sum death benefit" of $255.00 (currently) to eligible survivors of a deceased worker who meets the necessary requirements as determined by the Social Security Administration. |

| The lump sum death payment is made in "most cases" to the surviving spouse who was living with the worker at the time of death, if none, then to a spouse who is eligible for benefits, or a child or children who are eligible for benefits. |
| B. Benefits for Survivors
Monthly Social Security Survivor's Benefits may be paid to certain family members, who meet the necessary requirements as determined by the Social Security Administration. These include the widow or widower, dependent children and dependent parents.
Those who may be eligible for Social Security Survivor Benefits are: 1. Widow or Widower age 60 or older 2. Widow or Widower of at any age if caring for the deceased's children who are under age 16 or disabled. 3. Divorced wives and husbands age 60 or older, if married to the deceased 10 or more years. 4. Widow, Widower, Divorced wives and husbands age 50 or older, if they are disabled. 5. Children up to age 18. 6. Children age 18-19. if they attend elementary school or high school full time. 7. Children over age 18, if they became disabled before age 22. 8. The deceased workers parents age 62 or older, if they were being supported by the worker.

| Documents that Social Security may require in order to determine eligibility are as follows:
* Your Social Security number and the deceased worker's Social Security number * A death certificate (In most cases the form that we send to Social Security can be used for this purpose.) * Proof of the deceased worker's earnings for the last year (W-2 forms or self employment tax return) * Your Birth Certificate * Marriage Certificate (If applying for benefits as a widow, widower, divorced wife or husband.) * Divorce Decree (If applying for benefits as a divorced wife or husband.) * Children's birth certificates and Social Security numbers, if applying for children's benefits. * Your checking or savings account information, if you want direct deposit of benefits. If you do not have the original documents, or all the information needed, do not delay your application for benefits. Social Security will assist you in getting the necessary paperwork. We also suggest that you contact the local Social Security Office as soon as possible if applying for any children's benefits. |
| 2. Veteran Benefits
Federal Veterans Administration Benefits:
All Active members of the Armed Forces and those that have an Honorable or Similar Discharge from the Armed Forces are entitled to a U.S. Flag and for burial in a Veterans Administration National Cemetery.
The flag is issued by the Department of Veterans Affairs and at the time of death. The Geiple Funeral Home, Inc., will arrange to receive the flag for the family. Following completion of services the flag is presented to the first eligible surviving relative. The order of who is first eligible to receive the flag is prescribed by law.
For burial in a Veterans Administration National Cemetery, upon request by the family and upon receipt of the Honorable Discharge and Service Record the Geiple Funeral Home, Inc. will arrange for burial in a VA National Cemetery. Eligibility for burial is determined by the VA Cemetery and is subject to their scheduling.

| Other Federal Benefits |
| Headstone or Grave Marker Benefit:
The Veteran Administration will provide headstone or a grave marker for the graves of a veteran anywhere in the world and for an eligible dependent of the veteran who was buried in national, state veteran or military post cemetery.
Niche markers also are available for identifying cremated remains
The Veteran Administration does not pay the cost of placing or installing the headstone or grave marker in a non-national, non-military post or non-state veteran cemetery. |
| Burial Benefits and Plot Allowance:
A monetary burial and plot allowance maybe paid by the Department of Veterans Affairs if a veteran: * Died as a result of a service-connected disability * Was receiving or entitled to receive at the time of death a pension or compensation for military service * Died while hospitalized or domiciled in a VA facility or other facility at VA expense * Eligibility may also be established in some cases of indigent veterans whose remains are unclaimed.

| Veterans Life Insurance issued by the Veterans Administration:
If requested to do so, Geiple Funeral Home, Inc. can alert the VA Life Insurance Division so that an insurance claim form may be sent to the veteran's beneficiary.

| Survivor Benefits:
In some cases a widow of veteran may be eligible for survivor benefits. We suggest that you contact the York County Office of Veterans Affairs or a counselor at the nearest VA regional office regarding eligibility of survivor benefits. Addresses and phone numbers are listed below |
